PCN Nurse Counselling, how it benefits Diabetic patient?
Under the Primary Care Network (PCN), the provision of Nurse Counselling services and the close monitoring of diabetic patients on the Disease Registry will help to offer better care and improvement in diabetic patients’ medical condition. During the Nurse Counselling session, a detailed diabetic wellness plan will be developed according to the patient’s needs and patients will also be taught on how to set achievable goals to better manage their condition.The funding from MOH and AIC also help manage the direct costs of these services for our patients resulting from the available convenient locations and affordable prices.
