AcuMed Medical Group is pleased to announce that we have recently been awarded to helm "Assurance Primary Care Network" which is part of the Ministry of Health’s (MOH) Primary Care Network (PCN) scheme.
What is PCN?
The PCN scheme involves clinics organising themselves into networks that support holistic and team-based care. Patients who visit these GP clinics under the PCN scheme for follow-up check-ups for their chronic conditions e.g. Hypertension, Diabetes, High cholesterol can expect better coordination of care by Nurse Managers and ancillary services such as Nurse Counselling, Diabetic Retinal Photography and Diabetic Foot Screening.
How it benefits patients?
Providing the best care possible for the patients is our goal. However, in the private sector, we faced several constraints, including lack of resources, time and ensuring affordability of services for patients. By grouping together and forming a PCN, resources such as Diabetic Retinal Photography, Foot Screening and Nurse Counselling can be shared and accessed more easily by our patients. The provision of Nurse Counselling services and the close tracking of patients on the Disease Registry will achieve better patient compliance and improvement in patients’ medical issues in time to come. The funding from MOH and AIC also help manage the direct costs of these services for our patients resulting from the available convenient locations and affordable prices.
At AcuMed Medical Group, our patients’ health is at the heart of what we do and we are constantly seeking new modes of care where we are able to monitor our patients’ health more closely and improve on care outcomes.
